3rd headline: Poor Taste
Let's face it, ending a headline about a fatal crash: "Investigators Say Train Engineer Sent Text Message Seconds Before ..." is just plain poor taste.
The headline was simply restated in the body of the report, followed by the obligatory second and third headlines, essentially repeating the words of the first.
